What to Expect on your FIRST VISIT

After being greeted by one of our friendly staff members, you will be asked to complete a health history form for yourself, or your child (download the forms here to save time in the office). This will help us learn more about you, your life, and the goals you have regarding your health. Once completed, your personal appointment will begin.  This appointment is 1 hour in length and will include:
